You must first log in with your default credentials. We have assigned the below default credentials:
Default Username: First Name + Last Name + DOB (mmddyyyy)
Default Password: DOB (mmddyyyy)
This is based on data currently in Workday. You can check that your information is correct under your profile or your spouse’s information by selecting Benefits and Pay, then My Dependents. If you have previously participated in the program with Personify Health, your credentials will no longer work. Use the login instructions above instead.
